Family Case Management, funded by the Illinois Department of Human Services, provides intensive and comprehensive case management to pregnant women and their infants up to the age of one.

Our bi-lingual staff conducts outreach activities to inform expectant mothers of available services and to assist with locating appropriate prenatal and well-baby care.   Our clients are uninsured pregnant women and infants in families whose income is 200% below the federal poverty level, who receive Public Aid, WIC, or who are receiving medical care at the Board of Health. 

Family Case Management conducts workshops on various topics, including SIDS prevention, breastfeeding, and prenatal care.   We also have a nurse on staff to answer client questions.

At Family Case Management, we walk with our clients from well-baby visits through labor and delivery.  Our doula program is instrumental in this effort to help babies get a healthy start.  A doula provides physical, emotional, and informational support during labor and childbirth. Family Case Management serves approximately 1600 families each year and has been instrumental in reducing the infant mortality rate in Woodlawn and the surrounding communities.   

Our clients often have needs that extend beyond the services we offer.  We make every effort to connect our clients with the necessary services to ensure they have a successful pregnancy and their babies have a healthy first year of life.  Some of our referrals include:
